Bills and Fees
If your health benefits plan includes an office visit co-pay,
your insurance requires payment of this amount at the time
of your visit.
Each insurance plan has its own set of benefits and
limitations. It is your responsibility to know what is covered
under your health plan and what services or supplies might
be excluded. You also need to know the amount of any
deductibles, co-insurance, co-pays, and any annual or
lifetime maximums which may apply.
We will bill you directly for any charges not covered by your
insurance as well as any co-insurance and deductible
amounts. For your convenience we accept cash, personal
checks, Visa and Mastercard. There is a $25 fee for returned
checks. You may also pay your bill online by clicking here.
Outstanding balances must be paid within 30 days of billing.
If circumstances prevent you from paying your balance in full,
you must speak with the office manager to make acceptable
payment arrangements.
We are happy to complete school, camp, sports or
employment physical forms free of charge. These forms are
usually available for pick-up in three business days after
receipt. Should you need expedited completion of forms,
there is a $10.00 fee.
